I hereby consent to the processing by the Representative Office of Medical Web Technologies, Moscow,
legal address: 105425, Moscow, Sirenevy Boulevard, 14, Bldg. 3 (hereinafter referred to as the Operator) of the following of my
personal data (hereinafter referred to as Personal Data): full name, date of birth, place of work, home address,
contact phone numbers, email address, and any other information related to my identity, available or
known to the Operator at any given time, including receipt from me and/or from any third parties, taking into account the requirements of Russian Federation legislation. I confirm that by giving such consent, I act of my own free will and in
my own interests.
I give this consent for the following purposes:
- Obtaining reliable information about Medical Web Technologies products and new medications.
- Obtaining new Newsletters from Medical Web Technologies, including educational events in the field of healthcare.
This consent is granted indefinitely to any actions regarding my Personal
data that are necessary to achieve the above purposes, including, without limitation, both in written and electronic form: collection, systematization, accumulation, storage, clarification (updating, modification),
use, distribution (including transfer), anonymization, blocking, destruction,
cross-border transfer of Personal Data, transfer to the Operator's affiliates via the Operator's network,
transfer to government authorities authorized to request such Personal Data, as well as
any other actions with my Personal Data, in accordance with current Russian legislation.
This consent may be revoked by me at any time by sending a written notice
to the Operator. I hereby acknowledge and confirm that, if Personal Data is required to be provided to a third party to achieve the above purposes, the Operator has the right to disclose (disseminate) Personal Data to such third parties who will have the right to process the Personal Data based on this consent.
I hereby undertake to promptly inform the Operator of any material changes to my Personal Data.
